i. A working color printer.
ii. A piece of blank white paper that fit to your color printer.
iii. A computer installed with Notepad, Wordpad, MS Word, OpenOffice or any software that can process a word typing.

v. A list of fake doctors name and university where he/she has been study.
Steps of how to produce the doctor's excuse note:
As soon as you prepared all the materials and equipments that I've written before, what you have to do is to imitate the example of doctor's note image that given above. Every aspect of the doctor's note image like the font type, font color and the style you have to imitate accordingly in order to prevent you get busted by your employer or human resources department.
What you have to replace is the doctor name, clinic or hospital name, address and the university name in the blue font color (doctor stamp). But remember, the font type, size, color and style must be remains same as in the image above. Check it once again to make sure everything was in what we desire then print them out.
Now you have put your fake signature embedded with the blue font doctor stamp located at the bottom right of my sample of free printable doctor's excuse note. Then write down in the provided blank space the date you meet the doctor, how may day(s) you take a fake leave and fill in your own diagnosis or fake disease that you pretend to suffer.
